ATTENDANCE OFFICE: Please call the attendance office if your student will not be attending school. When leaving a message, please leave your student name, grade, unexcused absences lead to truancy. Students can have up to 7 excused absences; a doctor’s note is needed after 7 excused absences.

Clubs: Students are welcome to join clubs offered at HMS. At this time, Clubs offered are Gamer’s Guild, Drama Club, Equality Club , Yearbook Club, Study Club and Black Student Union. Please come to the office for a permission slip. All permission slips must be filled out and signed by a guardian before starting.
Student Pick-up: If you wish to pick your student up for early dismissal, please come to the front office and sign your student out. The office professionals will call the student down after they are signed out. All students must be signed out before release. Family 1, Family 2 and/or emergency contacts listed in skyward are the only people who can pick your student up from school.

Exclusion Day will be here in no time!
If you have not scheduled your appointment, please do so as these will fill up
fast. We do not accept appointments to avoid exclusion from school.
6th grade requirements include:
● School Physical Exam - An IL school physical dated on or after the first
day of school last year is due before the exclusion date. (A sports physical
will not fulfill the physical exam requirement).
● Immunizations: There are vaccines due now. Hepatitas B, Tdap and
MCV. Sometimes some student needs more. Please turn in a copy of your
full immunization record.
● Dental Exam - copy of state exam form due to the health office by May 15.
Any exam that was done 18 months prior would qualify.
7th & 8th grade requirements include:
● School Physical Exam - This is only due if you are were not here for 6th
grade.
● Immunizations: We need a copy of your immunization record. You may
required some for school. Please double check with your provider.
● Dental Exam - copy of state exam form due to the health office by May 15.
Any exam that was done 18 months prior would qualify.
Forms turned in to the HMS Health Office as soon as possible to avoid any delays.
